Evidence

A Task states authorized work and acceptance criteria. A Report records what was actually done, observed, and verified. Neither replaces current state: STATUS.md remains the operational authority.

Keep evidence references specific enough to reload only the relevant Reports on a cold start. Mark claims that need re-verification instead of presenting them as current facts. This makes a new session able to distinguish verified work from remembered context.
